You have hit on one of the challenges in employee reviews. Many times, managers tend to evaluate employees based on a personal bias. To completely eliminate bias from the review process, it's essential to focus more directly on the job itself.

Therefore, the real solution would be to ask only job-related questions.

If you are able to tailor the questions to the job, it really makes no difference whether the questions on the self-assessment document match those of the employee review document.
