Author Sexual Harrassement Policy Acknowledgement Forms

We have just created our employee manual. At a seminar that I recently attended, they suggested that not only should you have a sexual harrassment policy but you should also have a form that the employee signs to acknowledge that they have received and understand the sexual harrassment policy. Each time you have a staff meeting, this form should once again be handed out and signed. They also suggested having one for your non-violence policy. I have not been able to find any such acknowledgement forms. Do you have any suggestions? Do you have any such forms available?

I have instituted both of those policies and made a pretty generic acknowledgement.
"I have on this ______ day of _______ received a copy of the (Sexual Harrassment/Violence in the work place/employment at will-fill) -in blank as appropriate- I understand it is my reponsiblity to adhere to this policy. Should I have any questions I will notify the Human Resources Department.

I have had one incident that an employee refused to sign. Then in a meeting with a witness that has access to partial personnel records, I read the policy to him word for word and stated that on the form. Where I have the place for employee signature I wrote "Declined to Sign", and I also made a disclaimer- refusing to sign does not exempt employee from this and all/or other company policies.

Another approach, that I often use, is to make employees sign a "sign in sheet" with the topic listed, like "[X Co's] Sexual Harassment Policy and Harssment Prevention." During the program, materials are distributed to all employees. A copy of the material and the sign-in sheet can be placed in a single file for reference. Seems to work. (I have had occasion to use these sign in sheets in court and at depositions, and the employees really hate admitting that they signed it! "Well, it looks like my signature, but I do not recall it." "Well, Mr. Witness, as you sit here today, do you have any reason to believe that this is not your signature?")
