We are looking at continuing to offer Extended Health Benefits to employees on leaves (maternity, parental or personal unpaid leave of absences) and are wanting to know what the industry standard is for length of time. Tentatively we are extending these benefits (providing the employee pays the entire cost of the plan) and want to know if there should be a time limit set...for 2,3,or 6 months? Any thoughts or experiences?

If someone is on a LOA such as maternity leave and they plan to come back I don't think you can stop providing health insurance or any other benefit. Or are you planning on firing the person when she calls to tell you she is headed to the hospital and then rehiring her at a later date? If she is your employee and was on your health plan the day before she had the baby she should still be on your health plan the day she has her baby or whenever her maternity leave officially starts.

In the case of FMLA I believe again you cannot eliminate someone from your health insurance or any other benefits he/she were eligible for before they took leave.

I agree with the previous poster on benefits during FMLA-covered leave.
We do not offer extended benefits for folks requesting personal leave of absence. Most of our benefits participation is determined by the provider and would exclude participation by someone on LOA (not working). With respect to medical and dental, COBRA applies once they stop working, so they are in fact covered under our plan at their own cost, for 18 months anyway, whether for personal LOA or beyond the amount authorized by FMLA.
