Does anyone have a sample letter to send to an insured that their COBRA time period is ending? I have one coming up shortly and although I have talked to the insured, I would like to send the family something in writing.

Also..can COBRA be extended past the 18 month period? I am a little confused on this issue.

Yes, if the former employer so chooses, the coverage can be extended more than 18 months. We just did so, as a matter of fact. The law states that you must offer it for 18 months, but there is nothing in the law stating that you can't offer it longer if you choose.

While it's true that you can offer COBRA beyond 18 months, I'd pass it by the insurer before doing so (if it is a voluntary extension). Of course, certain dependents have 36 months to start with.

Disabled beneficiaries might qualify for up to 29 months of coverage. During months 19-29 you can charge 150% of premium. These beneficiaries had to be deemed SS disabled within so many days of their qualifying event and they had to notify you within so many days of their SS disabled status. I believe that the time frame is 60 days from QE but you'll want to check the regs if that is your situation.
