I recently completed a benefits survey at my organization using an online survey generation tool called Zoomerang (http://www.zoomerang.com). They have several built in templates that you can use to make the survey generation easier.

The template is more of a benefits satisfaction survey, so we went in and created questions such as "Please rate your level of interest in the following types of benefit programs:
EAP
Health Insurance
..."

Zoomerang will also keep track of the results for you.

HI skjones. I am an employee benefits broker/consultant in NJ. The us gov. just put out an extensive survey covering all types of benefits. Its very detailed. Do a seach under employee benefits us government on the web. A link should pop up. I do have other surveys my company has purchased.
