I don't think you're going to find a definitive answer. It will really depend on your organization and how paternalistic it is, and what your cash flow looks like. We are a small but growing internet security firm, and we pay 100% for our full-time employees' medical, dental, life, ad&d, and LTD premiums. Part-time employees (<30 hours) are paid at 50% for med/den and are not eligible for the other insurances.
