For our health insurance coverage, there's currently not really any verification of dependent status. They do send out forms to fill in for when dependents get to be around college age, to make sure that we're not carrying people who have aged out of moved on, but this is pretty much relying on the honesty of those surveyed.
Do any of your companies, or your health insurers require documentation to back up claims of dependent eligibility? If so, what form does that take, and how often do they require it?
For example, for initial coverage, do they want a photocopy of a birth certificate? Copy of a tax return? DNA testing? Umbilical cord blood?
For continuing verification, a copy of a school schedule? A report card? Web-streaming from the classroom?
Does or has anyone done an audit of this?
We're just trying to figure out what the common practices are, and whether we need to tighten up what we do.

My company recently started requiring dependent eligibility documentation of employees. For a spouse, the employee needs to supply a copy of the first page of the most recent Federal Tax Return; or a copy of a marriage certificate and proof of joint ownership; or a copy of the marriage certificate if married in the current plan year. For a domestic partner, they need a copy of the Domestic Partner Registry or an Affidavit of Domestic Partnership and proof of joint ownership. For a dependent child, the employee needs to provide proof of parenthood and proof of financial dependency. Proof of full-time student status is required for dependents age 19 and older.

Proof of parenthood would include a copy of the birth certificate, hospital record, or final adoption order.

You insurance carrier should be monitoring any dependents once they reach the age when being a full-time student is required to continue coverage. Typically, they'll check twice a year - once in the fall and once in the spring since most colleges are on semesters. This probably varies a little from carrier to carrier, however, I would check with them and see how they handle this.
