Just trying to get an idea of what other's do...

When an employees takes an LOA, do you require that they continue to pay the premium amounts that would normally be deducted from their paycheck, or the entire premium including what the company pays on their behalf?

We continue to pay them for 3 months. Beyond that, the employee must pick up the full cost.

If its not FMLA qualifying leave, we require the employee to pay 100% of the cost.

While on medical or industrial leave (including FMLA) they pay the normal monthly employee premium for up to 26 weeks and then we COBRA after that.

We consider the LOA as a separate event from any FML. If they work a part of the month or get paid for a part of the month (using sick leave or vacation time), we pay any premium that we would normally pay. If it is LOA with no pay happening, they pay everything. All elected self pay coverages and all coverage payments that the employer would otherwise be responsbile to pay.

For us, that is only the employee portion of health insurance and a life insurance policy.

If they do not pay, we give them notice and they are in danger of losing coverages.
