It's been a few years since I have worked on Exec Benefits, but I would check with the larger consulting firms such as Mercer, Watson-Wyatt etc. The latest one I can find online is Nov 2007 from Clark Consulting and it doesn't look to have that great of participation (18% of top 1000 companies)

Unfortunately though, the data might be a bit dated and with the way the economy currently is, a bit overstated. Companies are cutting back everywhere and there is pressure from a lot of Board of Directors to cut back Exec Comp and Benefits. I haven't seen much published on non-qualified plans since they tend to be in the background anyway.

As ruppert pointed out, Clark Consulting has some good pieces. If you register with them, you'll be able to peruse a number of publications on executive health benefits. Not much is changing in this realm, so if 2007 is the most recent publication on this topic, it should suffice for what you're looking for.
