Surveying HR customers

I work in the corporate office, HR for a large decentralized provincial government. We have been moving away from the traditional transactional-type services to achieve a more strategic service. We would like to ensure our large business units/divisions are truly aware of our services. We would like to do phone survey (just-in-time type) to identify gaps and also see where we had great success. Eventually, we will be sitting at the CEO's table to report back. Has anyone developed such a survey (all encompassing - the entire life cycle of the HR services. Or can someone point me to a site on the Internet? Thanks
