I am trying to find a good sample disciplinary warning letter. It's tricky because the employee is in California. Does anyone have any suggestions, web addresses to check out, etc?

I am trying to find a good sample disciplinary warning letter. It's tricky because the employee is in California. Does anyone have any suggestions, web addresses to check out, etc?

Definitely add the following :

1. Date & Time of the incident.
2. Nature of offence.
3. Why is it a indiscipline ? Quote company rule book,standing orders etc.
4. Give the person a chance to show cause & appeal.
