I would like ot have a copy or the regulation number of the law that specifies that doctors cannot charge patients for making out disabiilty forms. This is not allowed by law in the State of New Jersey from my understanding. I need the regulation for the employee to be reimbursed by her doctor.

You should probably consult with your company's labor and employment lawyer for the specific cite (I do not have it handy) or contact the state department of labor or workers compensation board for more information. If all else fails, contact your local office of the United States Department of Labor (who might be able to refer you to an agency that can help you).
