best practices on pre-employment drug testing

I am looking for best practices on pre-employment drug testing and how to handle if the candidate/new hire tests positive. More specifically, does your organization do the following:

• Rescind the offer
• Allow applicant to reapply - If yes within 6 months, 12 months et cetera

Things to note, we have zero tolerance drug policy.

In addition to the normal pre-test questionnaire that the collecting facility takes, we do a split sample for varification of positives and if the second sample confirms a positive we rescind the offer and will not accept another application from the candidate.
