termination notice- do we need to write the employee's social security number?

We have a termiantion notice that we have all termianted emloyees sign. It states they received their final check, last day, and reason for termination. We don't write the social security number but is it required?
Also do we need to make a copy of this to the employee and have him/her take it with them?

Why on earth would you put a SSN on a receipt for a final check? You have the check number, you have a pay stub and in your files you already have the SSN. If the employee cash's the check you even haqve a copy of the executed check.

As to giving a copy of this "receipt" to the employee. Why not? What possible harm could come from doing so?

We do give a copy to the departing employee after they sign it and no there is no requirement that their SSN be on it.

Some states require a formal termination notification, but off the top of my head, I forget which ones. I know in Texas that the employee can request one and the employer must provide it if requested. So we pretty much give a copy to everyone.

We are in California. I believe we are confused about writing the ss# because the sample form the "Term Notice Pursuant of Sections 1089 of the CA Unemployment Code"
it has the social number on it? like if it's a requirement?

Hi:
California employers must give notice of the state's unemployment benefits to ALL terminating employees--the basis of termination does not matter. The forms that I have seen all contain a space for SSN.
