Recertifying FMLA

We have an employee who is on FMLA intermittently and we believe she is abusing it. Originally her certification indicated she would need to miss time every 1-3 months; lately she's been missing 1-3 times per week. I'm in the process of putting together recertification paperwork but I'm wondering if anyone has a sample letter that they've used indicating to the physican what her attendance pattern looks like? I've read the regs that say we can include this, but we're unsure of how to do so to ensure that the physician actually receives the copy. Any suggestions?
