If I talk to an employee about her attendance issues and she tells me she has a major medical condition that is keeping her from coming to work, am I required to give her a medical certification form

I would and would also consider providing ADA forms as well. She has started the interactive process and it is up to the employer to take the next step. Now her medical issue may or may not fall under either FMLA or ADA, but you need to request more information to know for sure.

Rrupert, I am familar with the FMLA forms but what type of ADA forms are you talking about? Do you have samples that you could share?
