KY breaks and lunches

I know in the State of KY there is a law that states employees get a ½ hour break for anyone working 5 hours or more and a 10 min break for those working 4 hours.

Here's what I want to check:

1) See if there are more stipulations regarding time worked and breaks.
2) Does it apply to salaried and hourly TM's or just hourly TM's?
3) Can a person sign a waiver to not take a break?
4) Does a waiver even exist ??
5) Are there other labor related items for KY that mandate time off during work?
