Forms, in general, are to help ensure that the same appropriate information is gathered from each respondee. I'm assuming you are refering to job-structured screening/intervew forms. If so, it should be a two-part process. 1) For each job, completed by the supervisor/hiring mgr to record a) key requirements on the job, ksa; special work demands, i.e. shift, etc b)type of training, exper, etc that would indicate the candidate meets the requirements c) questions that will be asked of each candidate to determine qualifications. This last is the basis of 2) screening/intervew form for each candidate. Form should also summarize basis for hiring decision based on factors indicated. |
