You need to make sure that the exit interviews are conducted by human resources, not the supervisor/manager. Considering the manager may be the one to provide a reference in the future, it's not likely the individual would be willing to identify areas of concern.

Create some standard questions, such as why were you looking to leave the organization, and what was it about the offer you had that made you accept it.

Keep it positive but probe for feedback, just as you would in a recruitment scenario.

If the person conducting is sincere, you'd be surprised at how much information you may get back. The hard part is dealing with some of the feeback you might get, especially if it is negative or accusatory.

You need to use your HR investigative skills here.
