Does anyone know if we can have identifying information (name) on the voluntary EEO Self Identification form on an application?
Do we need to know who self identified? Or just that they did so? I'v

Does anyone know if we can have identifying information (name) on the voluntary EEO Self Identification form on an application?
Do we need to know who self identified? Or just that they did so? I've seen most EEO forms with the names included, but we are considering removing it. Thoughts? |

First, you don't want the EEO data ON the application. Not sure if you meant to state it that way or not. A separate form that goes with the app and can be removed prior to review is a good way to go.
What purpose would having the name on the self identification form serve? |

The reason the name is on the form is that the data is entered into the HRIS as part of the employee record. How else would you report all the characteristics of the employee? Job, age, race and gender are all reporting pieces of the information. |
