Our company, a small (120 employees) distributor in Vancouver, BC is thinking of implementing an employee referral bonus program. I'd appreciate hearing what other companies are doing. -- If amounts are staggered e.g. one amount for warehouse, one for office, one for management or a flat rate. and when the bonus is paid e.g. 50% after 3 months and 50% after 6 months. Thank you!

Our is also broken down into types of positions. Hourly lower level vs management, etc...we do 50% at hire and then 50% at 6 months.

I like kearstenweeks model with a payment at hire. If you encourage employees to refer friends and relatives, pump up the program, etc, you should have some kind of instant reward at the time of hire.
It doesn't have to be a portion of the hiring bonus, though. Invest a few dollars in gift cards in various amounts ($25/$50/$100), bring a grab bag of cards to the referring employee's desk and let the employee reach in and take one.
It solves the instant gratification thing very nicely.

In my agency, the referring employee gets paid after the new hire has been employed for 6 months. Depending on his job status, FT, PT, or substitute determines how big the bonus is. FT is $300, PT $200, Substitute $100.

No such program at current employer but have used successfully in previous jobs. Bakc in the IT sector we paid up to $2,500 for the referral of some hard-to-fill skill sets. We paid 25%-25%-50%.

Our policy is $1,000. First $500 paid when employee hired and second installment after 6 months.
