Hello, does anyone have a FMLA tracking system that is in Access or Excel that they are willing to share?

We are building ours into our HRIS system, but currently use an Excel spreadsheet that is very basic. We track location, name, type of leave, type of illness, dates that the leave began, when the employee returned, estimated time off, and then just have a section for notes, where we can note when we received the paperwork, what else we need, etc. It's very basic, but it meets our reporting requirements at year-end. We use the same basic set-up to track short-term disability, including a column for when STD pay began and when the 6 mos. is over.

If anyone else has any suggestions, I would love to hear them too!
