It's not a policy that's usually written on company policy format. If the agency presents a candidate to you who's already in your database, then typically you don't pay any fee. You might want to set some time parameters on it along the lines of whether or not the resume had been submitted to you in the previous 12 months from another source (which protects you in case another agency submits the resume within that 12 months).
Most every agency recognizes the "first in" principle - first agency/source to submit the resume gets the credit. If your agency doesn't recognize that, then find a new agency. Not like there's a lack of them out there.
You may want to edit any agreement you have with agencies to reflect a "first in" clause. The last thing you ever want is two agencies having a legitimate claim to one candidate who has been hired and having to pay more than one fee for the hire. Paying a fee is bad enough; paying double fees is an absolute nightmare but it has happened.
