You haven't given too much to go on!
What system do you need training on?
I'm sure that whoever supplies the system will be able to providee the training. Alternatively (and especially if it is a larger system) tehre will be specialist training consulting companies that will be able to deliver bespoke training for your specific staff & needs. (e.g www.dacg.com).

Training should always be evaluated. But what do you want to evaluate - how good the facilities & trainer was, or whether staff can do their job.
'Happy forms' - (tick forms after training) are usually used - but in my view they are too subjective and show no correlation to the users knowledge at the end of the session
