All the above said feaures are available in this product called EmpXtrack.

Considering you needs I suggest having a look at E-appraisal from Halogen Software (www.halogensoftware.com) I've had success with other Halogen products including 360 and have achieved a huge ROI. I'm piloting the appraisal product at the moment and the costs are very low. You can also get it externally hosted. This way is generally much cheaper than in house, optimises server set up for the application, and you can be up and running in days.

There is also a new rater add on system which allows management to gather their own feedback from other staff members on specific objectives.

As mentioned above I have had great success with web based system and have used Halogen since 1999 for on-line surveys and 360 reviews. Their hosting is excellent if you wish to go that way and their customer service is the best Ive ever experienced.
