When I managed the oeprations for a classroom-based and online training application center, are methods for gathering feedback/evaluations were two-fold. We asked for immediate completion of an online evaluation form, so we could get a "happiness" evaluation. This was to be followed by a phone call within two to four weeks in order to determine how effective the training had been. I admit, we were not as successful in actually making the follow upo calls as we had originally intended.

I suggest you read Trainig for Impact by Dana Gaines Robinson and James Robinson. This book explains how you can design, develop, implement, and evaluate training programs that have impact and are not done just for activity. Follow up calls or emails 3 weeks down the road do no measure your impact if the desgin and development of the course does not address the "gap"

I have found some helpful information in the "Training and Performance Sourcebook" by Mel Silberman and patricia Philips. McGraw Hill.
In the 1998 version, there are 2-3 chapters dedicated to on-line instructions. Check out the more recent versions to see if there is more for you to pull from.
If you haven't already, don't forget to go back to some of the classroom methods and see what can be modified for on-line. You may also want to look into how on-line focus groups are being used for customer satisfaction surveys and other topics. More and more we are looking at gathering information through this kind of interaction.
Good luck and I hope you share things you are trying out.
