I have been tasked with starting a mentoring program. Does anyone have a form that they issued to employee's who want to be a part of the program?

I have just been given the fantastic opportunity to design a training program for an expending Restaurant. Materials are in the process of being created but Im having trouble with the final measurement piece- tools to guide the process and follow up surveys to evaluate the program. Anyone have any great ideas or links I could search??
Thanks a bunch-

Could I get a copy of the slides as well please?

Also, does anyone out there administer some sort of test/quiz approximately 30 days after orientation or training?

Darlenelaw,

I strongly recommend follow-up coaching and reinforcement activities for all training. Reinforcement could take the form of a quiz or test (depending on subject matter) but I think that on-the-job observations are much more valuable for follow-up of any training that's meant to be put to use on the job. How well is the learner applying the lessons learned to day-to-day work? If the training involved learning a new process, is the learner still doing the process safely and in the recommended manner? Can the learner still explain the important why and how of the training? All these things can be determined by an observation and coaching session.

Your information with regard to the mentoring of engineers was most helpful as I do believe there are some peculiarities with the different disciplines that need to be addressed separately.

Thanks again!
