There is a fabulous book called 101 Sample Write-ups for Documenting Employee Performance Problems - a guide to progressive discipline and termination. The book includes a CD-ROM with all the samples that can be modified. The author is Paul Falcone. This is expecially helpful to new supervisors/mangager/HR folks. I'm a firm believer in NOT re-inventing the wheel.

I agree with HRPOB...this book is excellent. the one thing i stress in my organization is to reinforce the positives and to keep away from exploiting the negatives. PIPs used to mean the kiss of death in our organization, but i worked hard using this book and had the rumor mill packed away. Employees who are put on a PIP now have the opportunity to receive additional coaching, training, and mentoring to bring their deficient competencies up to speed. I can't emphasize enough how important it it to keep the conversation positive. Exploiting weakneses will only make them faulter more than they have in the first place.
