There was another topic out here a while back and the general consensus was that it was not really a great business approach. You might try to search ... Some things I remember:
(1) You have to define when PTO ends and leave starts --- there must be a maximum. If not then you run into issues with FMLA, ADA and other things that require absence accommodations. You also have to define when TOO much PTO becomes an issue for discipline. And how will you have any standard if you policy is "unlimited". Hard to term because someone is using too much and not getting their work done. Because it will happen.
(2) this is truly why STD/LTD income replacement plans were invented
(3) It will be great until the first employee abuses it. There will be a large lack of control OR your managers will approve some and not others and possibly get into discrimination/unfairness (legal or illegal) complaints. Who has the right to say no to PTO and when?
(4) I have found the lack of setting boundaries to always cause an issue not too far down the road.
(5) there are different issues for Exempt and Non-exempt.
Maybe I am just too cynical for my age. But I have always found that being too generous often comes back to bite you.

Yep, it was called "take what you need PTO" on 5/18/11 about 5 pages down......you might read through that posting. The OP asked a bit deeper of questions, but there are some good perspectives.
