Hello,
In Indiana, are employees required to complete a wage deduction authorization form for medical/dental deductions? Wouldn't the signed enrollment form that they are enrolling in the plan suffice as authorization to deduct? Thank you.

Check your state wage and hour laws for the answer to this one. Many states have a law that requires an employee's specific authorization to make any deductions from wages.

You might also want to have on record the employee's acknowledgement of the strings attached to a Section 125 plan - having a form that authorizes both the deduction and the Section 125 considerations is a good way to do that.

Whoever is administering your Section 125 plan should be able to provide you with a copy of the policy.

Our enrollment form includes the authorization to deduct along with the Section 125 caveat about only being able to change with a qualifying event.
