Has anyone stopped paying employees for unused sick time and experienced an increase in sick usage because of it? Will eliminating the incentive actually backfire?!

Well, we have never paid for unused sick time...but do allow the employees to use it for things like dr's appointments, children's illnesses, etc.

You might think about that...

Other options could be to pay out some portion or to give a small stipend for each unused day, but not the full value of the day.

But yes, I could see certain populations of entitled employees doing exactly what you state. You could also require a doctor's note for sick time off, but that would probably be overkill.

Combining sick with vacation into an overall PTO account tends to reduce sick time taken since every day of sick time taken is a day less that could be taken as vacation.

The downside to doing this is that your payout of unused PTO upon termination will increase (if that's what you do).
