Strongly suggest you check with a local attorney who is very familiar with your state laws. Depending on whether you want a non-compete and/or a non-disclosure clause, those are VERY dependent on state law.
This is NOT a DIY project, nor would a sample contract really help as the details will be VERY different in every different situation.
However, some things to think about are change in control provisions, notice provisions, severance provisions, benefits promised, noncompete/disclosure, etc. You want to make sure the contract works for both sides and protects both sides where it needs to do so. And only someone very familiar with your business and state laws can do this for you.
