Contact your WC carrier. They'll be happy to advise you and have recommendations if not actual sample forms.

You might also check to see what exists at your state level WC department. Some are better than others, but I know TX has some specific required forms.

Also make sure you have a job description with physical requirements that the doctor can review....otherwise I have found doctors to give a very vague fitness for duty letter. They tend to do better if it is a WC injury where they are reporting to both the employer and the insurance carrier.

But for non-work-related injuries, I don't tend to get the same level of quality reports.

I would require the same (consistent) process for any injury/sickness, as long as it doesn't overstep ADA?FMLA boundaries. Usually if you require it for non-ADA/FMLA related cases, you can require it for ADA/FMLA cases (except for some recertification timing issues).

Definitely work with your WC carrier. They will make sure you're in compliance. Once you have the return to work program in place, you can start tweaking it so that it's tailored to your business.
