FMLA forms in Word format

Hi Everyone,

Our company recently implemented a new Leave system and we need to upload the FMLA forms into the system however they need to be in Word format. I of course have them in PDF version and I am having a bit of a hard time finding the generic forms in Word format.

If anyone out there has those forms in Word format I would greatly appreciate it if you would send them to me.

Can I ask why Word? I am not sure you will find them in Word format only because then they would be easily changed.

I know the DOL has the forms as PDFs where the answer spots are edittable. I can't imagine anyone would have converted them back to DOCs. Because I would not send one to an employee or dr that could be changed.

I do have a leave request form in an Excel format. But when it comes to the actual certification forms, I use the standard DOL ones.
