It has always been my experience, that employees are not only required to complete a Day Off Request Form for Personal and Vacation days, but also for any sick days they use. Isn't it important to document sick days?

Yes.  Especially so if there are limits on paid sick days that may be taken or to account for FMLA leave time.

Yes, it is important to have the documentation, but it doens't always have to be a form. Depending on how your company operates or systems work, many timekeeping systems allow for specific coding for sick time. 

The difference between sick time notification (done the day of or filled out afterwards) and vacation/Personal is that more notice is usually required on vacation/personal.   For example,  there shouldn't be a reason someone is asking off for a sick day 2 weeks in advance. So you will need a different procedure or policy to account for sick time. It could be a form or some other process. But yes, you do want to track it, even for exempt employees.  Especially for FMLA purposes.

Sick Leave is sometimes requested in advance, when it is for the purpose of scheduled medical exams or treatment or for dependent care for someone suffering from a long-term illness.
