Our organization is restructuring the performance appriasal process. We want to include an employee self-assessment section and are looking for an input mechanism to achieve this. We would want ees to list accomplishments, activities involving personal and professional growth, examples of excellence in customer serivce, how they moved the association forward, etc.. Any information would be most helpful. Thank you!

There is a simple way to get self-assessment from the employee as well as a more effective result. Try HOW-GOES-IT, a pratical (read dumb engineer's approach) to making performance appraisal truly effective. H=G-I also takes the sting out of the process and provides useful input to the supervisor.
