We are considering changing from our current vacation system, where employees are granted vacation time at the beginning of each year, to an accrual system.

I'm concerned about how to accomplish this. For example, if they use this year's vacation by EOY and they start accruing so much per pay period starting in January, they won't have enough to use if they need time off early in the year.

How do other companies handle this?

Some brainstorming.... I can't remember if your state is a "use it or lose it" state or not.

You could front load a portion accrued, say the first three months for the first year to avoid the zero balance issue. Or even accrue at a faster rate (so that they received all vacation available by mid year to be used by the end of the year).

That brings up another point in this change -- if you have a "use it or lose it" policy that goes by calendar year, you might have a lot of employees who save up throughout the year and then all the sudden want to take all vacation at the end of the year. Or who earn an accrual right at the end of the year and can't use it. Something to think about. One solution is to expire it based on the accrual date. But you'd have to have a transactional dated system to keep track of when it was accrued and when to expire it. Another solution is to allow them to use it up to x months past the end of the year.

We do give our employees their balance at the beginning of the year, but if they terminate, we calculate the termination payout prorata. If they have used more than the prorated amount, they get nothing more. If they have used less, they get the difference in their final paycheck. I very rarely have people who have used more.

We have an accrual system, but allow people to go up to 5 days negative, (assuming they will not exceed their total annual accrual).

My company switched for Vaction/Sick (granted at the beginning of the year) to a PTO system based on accrual in 2005. It has worked well for us. Another difference is that it is based on anniversary date, not calendar year. At 6 months the associate begins accruing PTO. Their PTO balance is on their check advice. They may at any time (other than while on leave) go into the negative up to 40 hours. On the flipside, they may carry over no more than 80 hours into a new anniversary year.
