I am trying to beef up our discipline and performance processes, including possibly making some changes to our "Employee Conference Report" form. There is, of course, a place for the supervisor to sign and the employee to sign. As we all know, employees frequently refuse to sign - sometimes they disagree with the statements and sometimes they are just angry. Is having the supervisor notate the refusal really sufficient? Would it make sense to add an option for the employee to initial a "refuse to sign" box....I think we already have a place for employee comments.

An employee might well refuse to sign just a signature space. A statement to go along with it (if you don't have one) might get you better results - something along the lines of "My signature indicates acknowledgement of receipt of this letter and does not necessarily imply my agreement with the contents of it".

If they still won't sign, a witness to the discussion would be the next best to have. If that's not possible, and it may well not be, then simply noting that the employee refused to sign is probably about all you can do.

is it possible that they refuse to sign because they have a different perspective about what happened? is it possible they refuse to sign because they have witnessed negative consequenced when others have signed? Sometimes power is misused.

If they didn't sign the original document then what makes you think they will sign the document to acknowledge they have refused to sign?

Norks suggestion is tried and true. It works just fine.
