An employee used FMLA not to long ago for their own serious health condition (FMLA only applied). They now may need time off to handle care for a seriously ill family member. In this case they will have FMLA and NJ FLA running concurrently, and the NJ FLA will extend beyond the time remaining for FMLA. How do other companies handle documentation? Do you simply make notation on the usual federal forms for FMLA? Does anyone have forms available (either provided by the state or a template you have created) for tracking NJ FLA?
