-Taking more than an hour lunch (about 1.5 or 2 hrs). Easy to keep records on.
-Distracting the student workers with constant chatting. When you see it, log it.
-Dressing unprofessionally. Easy to take a picture of with Cell phone each time it occurs.
-Taking directions from her mother, without consulting me. How are you determing this?

Can they get me in trouble, even when my director and assistant director were aware of these issues?

If you just keep doing your job at a high level you will likely be fine.

Daily Q&A
How to Address Flagging Motivation?
How do I increase motivation levels in the department? How do I brand my business unit as an attractive place to work? I have top-notch IT professionals in my business unit who feel they are "children of a lesser God" because they are non-billable resources and do not get plum postings abroad, nor the glamour that goes with them. As a result, their motivation suffers.
