Cut your losses and move on. Many of us have learned the hard way that extending the tenure of an "already proved it to the boss" bad employee is typically an exercise in futility. You have the perfect opportunity NOW to terminate this individual; take the emotion out of it and do it. Six months from now you'll hopefully have a productive individual in that seat and your stress level will be greatly reduced. What keeps this whole cycle going is your willingness to tolerate it longer....so Just do it! Good luck.

I agree with several of the people above, this is too lengthy and way too personal. You need to get the references to men and women out of there, you're just asking for a discrimation lawsuit. I also agree that the counseling thing is a great idea, but you're opening yourself up to a lot of negative reaction. I would suggest the EAP but not require counseling. If you are really trying to work this out, then make this the last warning and make it stick. Don't change your mind if this person is late again.

I have a general rule about written reprimands. If someone outside the company can't figure out what the heck the employee did then it is not a good reprimand. I have ready your written warning and for the life of me I can't figure out exactly what is going on here. I agree with the others who have posted - edit this down, take out the personal "trust" stuff, stick the the facts and document what the problem is (be specific) and what solution you want (be specific). Good luck
