If a supervisor ask a personin an accounts payable divison to set up an vendor and pay an invoice without proper documents and the employee refuses to do so because of the lack of the proper document has the employee commited insubordination? The employee informs the supervisor that the company policy is not to make payment without proper documents.

The employee is following proper procedures, so no it is not insubordination.
