What do you indicate on your evaulation form when you are interviewing a candidate that has the skill requirements for the job, but there is something about their personality that makes you certain they will not be a good fit with other employees in your office.

As long as the reason this person won't fit in isn't related to things like his/her race, religion, age, disability or gender, you can safely note this on the evaluation form. I've written things like "abrasive manner, may offend coworkers" and "doesn't display enough self-confidence for a leadership role" and those are perfectly OK things to note and record.
