Employee feedback on Management performance

I've been assigned evaluating 360 degree, have read postings which are mostly negative.   I did a survey of my direct reports by creating an evaluation form where the supervisor was rated and had good results.  I summarized the evalations to obtain an average rating of each category then rated the individual on their strengths and weaknesses and shared the scores individually with each leader. 

If 360 isn't working, what other methods are being used to get some honest feedback on a leaders performance?
