Morale at our startup company is low. I would like to have management send out a survey in order that they are aware of the grim atmosphere. We have had 12 people quit in the last year, which I think is very high for a company with has less than 75 people. Any ideas or places to go for a model of a good survey?

There are various surveys on this site. One example would be:

Don't bother having a survey unless you are prepared to take the findings seriously.

After all what if everybody says that supervisor X (the best friend of the boss) is a mongrel and treats people like dirt? Are you really going to deal with this or ignore it an hope it goes away?

After the first survey is done with no results you will never be taken seriously again. At the very least let the people surveyed know the results and why you cannot/will not act on the findings.

You may even get cost neutral solutions you hadn't thought of and which will completely address the problems.

Never ask employees questions if their answers will be ignored.

How many supervisors are there?

What is each supervisor's employee turnover?

Be sure to know if good employees are leaving or if underperformers are leaving. The first case is bad turnover, the second case is good turnover.

One good source for surveys is KnowledgeWave (Knowledge-Wave.com I think). They do employee and customer satisfaction surveys and do an excellent job, especially in coaching companies up front with what to expect and how to build surveys, and also on the back end when it comes to analysis and action.
